75 minute appointment
During your appointment we will discuss:

We will set one goal that is SMART (specific, measurable, achievable, realistic, and timely). I will conduct a Bioelectrical Impedance Analysis of your body composition to give me an idea of what your metabolism is doing. I like to give at least a couple of weeks to a month for follow-up appointments, depending on your specific plan, for you to implement what we discussed and to figure out what is working and what you are finding to be challenging.
60 minute appointment
We will review our prior appointment and check in to see what was implemented and how you are progressing. We will address any challenges and make adjustments to your nutritional and lifestyle plan as needed. We will review your past SMART goal(s) and discuss any barriers you experienced and your overall progress. and then set a new SMART goal. As you progress I will reanalyze your body composition with a Bioelectrical Impedance Analysis to check how your metabolism is improving.

HIPAA compliant virtual appointment (telehealth) allows us to easily connect online. An all-in-one video, audio, screen sharing, file sharing, and chat virtual appointment. You can connect without having to download any software, ensuring that appointments start on time, every time. By clicking on a weblink and entering a code, you are taken directly into your virtual appointment. It’s simple, secure and stress free. The video conferencing feature is compatible with both Mac and PC computers via the following browsers – Google Chrome, Mozilla Firefox. Android and Windows devices (tablets and smartphones) supporting the previously listed browsers may also be compatible. At the current time we are unable to support Safari, Microsoft Edge, or Internet Explorer. These browsers are not compatible with the infrastructure of the video conferencing feature. Likewise, iPads and iPhones are also incompatible. We are sure following future updates to Safari and Apple products, this will change.
Telehealth may not be approved through your insurance company. You are responsible for any amounts not covered or payable by your insurance. If your insurance denies any part of your claim, you agree to be responsible to pay the full balance. All services must be paid for at time of render. A superbill can be provided at your request for you to seek reimbursement. We provide a 25% discount for private pay. Telehealth appointment is at a discounted rate of $96.

Getting Ready for Your Visit
In order for me to provide you with the best care possible, it is very important that you bring the following items to your visit:
- Your photo ID
- Insurance card
- A list of your current medications and supplements
- Any and all lab results in the past 12 months
- Information about treatments and medications tried in the past
- Medical records from prior visits that you think may be helpful
You agree to keep all scheduled appointments and be on time. If you cannot attend a scheduled session, you will call to cancel and/or reschedule. There will be no fee if phone message or conversation is received before 24 hours of the scheduled appointment time. You understand if you miss or cancel with less than 24 hours of notice, then you will be charged for the full price of the appointment.
You understand that you are obligated to ensure that our fees are paid in full. We will verify your coverage and bill your insurance carrier on your behalf. However, you are ultimately responsible for payment of your bill. You agree that you will pay any deductible and co-payment or co-insurance as determined by your insurance plan. Those payments will be due at the time of service. Many insurance companies have additional requirements or stipulations that may affect your coverage. You are responsible for any amounts not covered or payable by your insurance. If your insurance denies any part of your claim, you agree to be responsible to pay the full balance.
Please let us know if Camille Redmond, RDN, LD is not a preferred provider on your insurance so we can apply for credentialing and contract with them. However, all services must be paid for at time of render. A superbill can be provided at your request for you to seek reimbursement. We understand that protected health information about you and your health is personal. We are committed to protecting health information about you. To promote quality care, Nutritional Path to Wellness operates an electronic medical record (EMR) Kalix. We contact you (by email, telephone, voice message and/or text (SMS) message) as a reminder (including automated reminders sent via Kalix EMR and Practice Management Solution) that you have an appointment for treatment or medical care. We may tell you about or recommend possible treatment options, treatment alternatives, or health-related benefits or services that may be of interest to you. If you give your consent to use electronic documents, you can later change your mind and request a paper agreement instead.
